Taking certain antihistamines with bisoprolol may lead to an excessively slow heart rate or a drop in blood pressure, which can cause dizziness or fainting. Additionally, some older antihistamines can slightly interfere with how well bisoprolol works to control your heart rate.

Why this happens
Bisoprolol is a beta-blocker that slows the heart rate and lowers blood pressure. Some antihistamines (particularly older 'sedating' ones like diphenhydramine) have 'anticholinergic' effects that can oppose the heart-slowing effects of bisoprolol. Conversely, both types of medicine can cause drowsiness and low blood pressure, leading to an additive effect where you feel much more tired or dizzy than usual.

However, you should avoid older, sedating antihistamines (like diphenhydramine or chlorphenamine) unless specifically advised by your doctor.
